Genetics, mostly. But socialization, culture, etc possibly. There have been trans people as long as there have been people. Some cultures were more accepting than others. I’d encourage you to check out Native American Two Spirit people for an ancient way cultures of the area perceived some kinds of trans people, often in a quite positive way.
